Our Frailty Team are there to support our doctors with the care of those patients who are over 75, live in a care home and particularly those at risk of admission to hospital, who are able to visit patients at home on behalf of the GPs. The nurses are able to assess and agree treatment plans with the GPs, enabling the patient to be cared for at home where possible.
