Registrations

YOU WILL BE ASKED TO PROVIDE SOME FORM OF IDENTIFICATION UPON REGISTERING.

The following documents will be accepted:

  •     MEDICAL CARD WITH NHS NUMBER
  •     BIRTH CERTIFICATE
  •     MARRIAGE CERTIFICATE
  •     CURRENT UK PHOTOCARD DRIVING LICENCE
  •     CURENT FULLY SIGNED PASSPORT
  •     NATIONAL INSURANCE NUMBER CARD
  •     LOCAL AUTHORITY RENT CARD
  •     CURRENT RESIDENCE PERMIT

We require all new patients to fill out our new patient health questionnaire before registering at the practice. This provides us with a detailed account of the patients’ blood pressure, height, weight, smoking status and alcohol consumption. It also tells us whether the patient has any underlying medical conditions such as asthma or diabetes and allows us to treat the patients appropriately in the management of their chronic disease. It usually takes several weeks for your medical records to be transferred but we are able to request an urgent transfer if necessary.

 

Carers

If you are the carer of a patient at the practice, please inform one of our Receptionists. You will be given a carers pack and advised about the services provided locally for carers

Temporary Residents

We have a policy where we are unable to see patients who are not registered at the practice unless they are:

  • Staying with one of our patients
  • On holiday in the area
  • Significantly unwell and need immediate treatment

As a rule, we do not visit temporary residents; however there may be exceptions to this.

Overseas Visitors

All visitors from overseas with non-emergency conditions will be seen privately. Please ask Reception for further details of this service.
