New Patient Registration

We require all new patients to fill out our new patient health questionnaire before registering at the practice. This provides us with a detailed account of the patients’ blood pressure, height, weight, alcohol and smoking status.  It also tells us whether the patient has any underlying medical conditions such as asthma or diabetes and allows us to treat the patients appropriately in the management of their chronic disease. 

It usually takes several weeks for your medical records to be transferred but we are able to request an urgent transfer if necessary.

Overseas visitors

All visitors from overseas with non-emergency conditions will be seen privately. Please ask Reception for further details of this service.

Non-English Speakers

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.  Please allow 2 working days for your form to be processed.  You will need to visit the Practice to sign the form and provide proof of identity to complete your registration.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
