CHRONIC DISEASE NURSE

You have a unique opportunity to join our high achieving, forward thinking Nursing Team in shaping the future of primary care.

We are a friendly, busy, growing GMS practice in Melksham, working from a purpose built Medical Centre. We have a multi skilled team with a wealth of experience and are looking to welcome a Chronic Disease Nurse.

Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients.
The post-holder will work with reference to Practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with the Nurse Manager, Nursing Team and the GP Partners.

The post-holder will have the required level of training and competence and will work within patient group directions where appropriate, following practice policy, Meds Mgmt, NICE guidance and current legislation.

You will be fully supported by a broad clinical team comprising of GPs, Clinical Pharmacists, Advanced Nurse Practitioner, Practice Nurses, Health Care Assistants and Phlebotomists.

It is a permanent role. Full induction and training is given. We offer a generous NHS pension scheme, staff uniform, five weeks annual leave and bank holidays pro rata.

Nurse Manager     
4-5 days a week
Salary: £42k to £45k pro rata

We are a friendly, progressive and well-established GMS practice in a thriving, growing market town. Situated just 20 minutes from Bath, our modern, purpose built building is in a great rural location, with excellent rail and road links to the M4 and London.

We have a professional, committed team and now require an enthusiastic, highly motivated and dynamic Nurse Manager to provide leadership to the Practice Nurse Team, in collaboration with the GP’s and Practice Manager, and develop a quality driven Nurse service to our increasing patient population.

   

Primary role:
  • Chronic disease management, ideally asthma, diabetes and COPD
  • Nurse triage, face to face and telephone
  • Development of clinical policies and procedures
  • Management of nurse led QOF areas
  • Infection control lead
  • Lead for annual seasonal vaccination programme
  • Management of a skilled, experienced and diverse nursing team

CLINICAL PHARMACIST
Salary: circa £35,000-£41,000
Spa Medical Centre & Giffords Surgery – Melksham

This is an exciting opportunity to join our forward thinking practices, working alongside our Senior Clinical Pharmacist as part of our multi-disciplinary team.

This role is focused on collaborative working across 2 practices: Spa Medical Centre and Giffords Surgery. You will work closely with GPs, Nurse Practitioners and our older person’s clinicians as part of a multi-disciplinary team.

The ideal candidate will therefore be a forward-thinking, enthusiastic individual with initiative, keen to work within a team of like-minded people.

Clinical pharmacists work in general practice to provide specialist expertise in medicines.

A patient facing role, the pharmacist will be expected to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas. They may be prescribers, or training to become one and must work with and alongside the practice team, to proactively support the management of people with complex polypharmacy, especially the elderly and those with multiple co-morbidities.

Ideally you have at least 2 years clinical experience, be able to prescribe and or have completed a clinical diploma or similar or be working towards these so as to provide specialist expertise in medicines use and optimisation in this patient facing role.

In addition, there will be a funded training programme for independent prescribing and clinical supervision will be provided.

Our practice area is in Melksham, Wiltshire covering a combined list of 25,000 patients; both practices have a Good CQC rating.

Up to 26 hrs per week available – Salary dependant on experience

THEATRE ASSISTANT PART TIME
(25 hours minimum per week, flexible) 
Salary: Hourly rate £8.85 (£17,158 WTE per annum)

Spa Medical Centre is looking for an enthusiastic & motivated part time Theatre Assistant to join the Surgical Services Team. Working as part of a dedicated team, the successful candidate will be assisting in both theatre administration and providing quality patient care, under the guidance and supervision of the Surgical Services Clinical Lead and Surgical Services Manager delivering quality surgical services, working to set policies and procedures.

Immediate start, full training and induction programme provided.

Essential Qualifications and Experience:

Working towards or in receipt of CQF Diploma level 2/3 (not essential)
Excellent communication skills both written and verbal

18:30-21:00 Mon to Thurs & 18:00-20:30 Friday

We are looking for an enthusiastic, hardworking cleaner to join our Team working five evenings a week. It is a part time role with flexibility to work additional hours to cover holidays and sickness. You will work with a supportive, experienced and friendly team. High standards of cleaning are required to meet infection control guidelines and provide a clean, bright environment to service users.
